  • Information Required for any Transcript Request

    • Current name and any other name used while attending Southwestern
    • SAGU ID number or Social Security Number
    • Date of birth
    • Daytime phone number
    • Current address
    • Dates of attendance and/or degree(s) received
    • Address(es) where transcript is to be mailed (up to 5 per week may be requested)
    • Student's written signature
  • How to Order

    • Download request form. Click here to download. Note: The form above must be filled in completely. A separate form must be used for each different address to which you are requesting a transcript be sent. Print out the form and sign where indicated with your hand written signature. Printed, typed, or computer signatures will not be accepted. Once completed, fax or mail the form. Do NOT e-mail the form.
    • Former students may also complete a transcript request and pay online at www.getmytranscript.com. This is a secure service offered through the National Student Clearinghouse. Please Note: In compliance with federal FERPA laws, requestors will need to fax in the signature page before the request is considered complete.
    • Students with an mySAGU portal account may login and select "Academic Records", which will take them to a secure site through National Student Clearinghouse.

To Request a Transcript in Person

To order a transcript in person, please come by the Office of the Registrar located in the Davis Building and complete the official transcript request form.

To Request a Transcript by Mail

To order a transcript by mail, send the required information in a letter, or use the request form. Mail to:

To Request a Transcript by Fax

To order a transcript by Fax, send the required information, or use the request form, to Transcripts, Southwestern Assemblies of God University, (972) 923-8159. Be sure to sign your fax and include a telephone number where you may be reached if we have any questions.

Holds

Transcripts will not be released for students with outstanding balances owed to Southwestern Assemblies of God University; for students who have defaulted on student loans; and/or for students who have not completed exit interviews for Student Financial Aid. Other holds may prevent the release of a transcript. If you are a current student, you may view your holds through CampusWeb. Contact the Transcript Office at (972) 825-4640 if additional information is needed regarding transcript holds.

Transcript Preparation Time

Transcripts will be mailed within three to five business days from date we receive the request for $7.00. Transcripts may also be mailed within one business day from the date we receive the request for $12.00. You may request additional copies (during the same request) for $5.00. There is an additional fee of $1 to have transcripts faxed.
