Registration Forms
Below are the forms that every Discipleship Ministries student will need to complete in order to begin the registration process for each semester. Once forms have been received, students can complete the registration process online through the CAMS Student Portal.

- Grant & Course Selection Form (Spring Only)
- Grant & Course Selection Form (Fall Only)
